2009 REGISTRATION

Parent Survey Comment Our 2009 LMT Safety Town program will be held at Session 1 July 6-10 9:00 am -- 12:00 pmTA Session 2 July 13-17 9:00 am -- 12:00 pm SESSIONS 1 & 2 ARE FULL. We are waitlisting. Please see below. We have availability in our other sessions
at this time (06/23/09). PayPal is not available for waitlist requests.
Session 3 July 20-24 9:00 am -- 12:00 pm Registration is now closed. Also see below for details on confirmations, change policy, shirts, helmets, etc.... etc.... Please be sure to read the form carefully and fill it in completely before submitting it. Incomplete forms will not be processed.
REGISTRATION UPDATE 7/15: REGISTRATION IS NOW CLOSED. Final confirmation postcards will go out Thursday 7/16 for those registered by July 15th.
***Please note that in order to participate in this program, your child must be at least 4 years old by January 1, 2009 and must not have attended kindergarten. This is a pre-school program.
REGISTRATION FEE AND PAYMENT: Our registration fee is $80. You may register online and pay via Pay Pal or you may submit a check made out to LMT Safety Town with a completed form (see pdf version below) via regular mail You don't need a PayPal account to pay via PayPal. PayPal safely allows you to pay online with your credit card or check, or after signing in to PayPal with an electronic bank account transfer or cash in a PayPal account. Please note if you register online: You must submit a completed hard copy of the Parent Signature page (below) if you register on July 15th or later please bring your complete Parent Signature Form to your child's first day. Please use the link below, not the second page of the online form. It is not meant to be printed out.
REGISTER ONLINE: Click Here Please be sure to complete your PayPal payment and get a confirmation from PayPal (you will also get an email confirmation from PayPal after the fact) AND hit submit on your registration form which will take you to our Thank you for registering...page for your registration to be accepted.
If you registered online please complete the following: Parent Signature Page - Please print, complete, and mail the parent signature form if you registered online. Registering online holds your spot (based on availability),
however we do require the Parent Signature page before June 15th so that it can be
processed into your child's confirmation before his/her session. If you register your child
after June 15th, please mail it ASAP to reach us before your child's first day. Registration is
not complete without this form.

WAITLISTING: Waitlisting is not applicable at this time. If it should become applicable and you would like to be placed on a waitlist for Session 3 please print out and complete the printable form, circle the session you would like to be waitlisted for, if more than one note your 1st and second preference, write waitlist on the top of the form, and mail the completed form in with the registration fee. We cannot accept PayPal payment for waitlists. If you do not clear the waitlist your original check will be returned to you on or before August 10th.
SHIRTS: On the first day of your child's session he or she will receive one LMT Safety Town shirt which must be worn every day and a LMT Safety Town water bottle. You will have the option with online or hard copy registration to purchase an extra shirt for $10. Extra shirts are not available after June 13th. Additional shirts must be paid for at time of registration and will be given to you on the first day of your child's session.
REFUND/CANCELATION POLICY: All fees are non-refundable and non-transferable.
CHANGE FEE: There will be a $10 non-refundable change fee for any and all changes. Changes are based on availability. No changes will be made after June 15th. Change fees must be paid via check and will not be confirmed until check is received. To make changes after registration send an email to registration@lmtsafetytown.com we will confirm whether your change is possible and inform you how to proceed.
REQUESTS FOR SAME CLASS: Unfortunately, we are unable to guarantee requests for friends to be in the same class. We will guarantee requests for siblings to be in the same class, however they will not be in the same pod within the classroom.
REGISTRATION CONFIRMATION: We are unable to respond directly to individual requests for confirmation of receipt of registration forms. Registration forms will be processed in the order that they are received. Confirmation postcards will be mailed on the 15th and 30th of each month (beginning in April). This will be your one and only registration confirmation so please read it carefully and keep it in a safe place until July. It will refer you back to this website for detailed information, updated regularly, to refer to at any time.
BIKE HELMETS: Please be sure that your child's helmet is adjusted properly before his/her session begins.
LAST DAY OF EACH SESSION
ADVANCE NOTE TO PARENTS:
On the last day of each session parents will be invited to come and see what their children have been learning.
Unfortunately, due to safety concerns and space
constraints we will be unable to accommodate siblings
or other relatives. Thank you in advance for your
understanding and cooperation with this matter.
